type of gallery you own / operate. Original artwork is not
available to all galleries. Consignment of artwork is account dependent.
We ask that you complete the following online forms.

If you are a Gallery / Dealer looking to carry the artwork of Dale TerBush, please complete this form. Be sure to add photos. All questions are required to be completed to expedite the process. Not all submissions will be accepted.

  • Please submit a scanned copy of your resale license. Formats accepted are: pdf, jpg, jpeg, gif, png. Minimum resolution if in jpg, jpeg, gif or png format need be 200 dpi. This is required for us to have in order to submit for tax purposes.
  • Please enter the date in which you first opened at the current physical location address.
